2/18/2024 EnchantaVaughn Approved Behavior: When QMHP arrived at client’s residence, family welcomed her back and was very polite. Client was observed helping mother in the kitchen set up chairs for the session. Client demonstrated positive affection towards her counselor. Client appeared to be happy as evidenced by client smiling. Client displayed appropriate affect and easy-going behaviors as she engaged in an appropriate manner with the QMHP. Client displayed her ability to use positive communication as she expressed her feelings and concerns at which her behaviors lasted throughout the session. Behavior: Client showed control over any intense angry feelings. Mother reports when she cannot have her way within the home setting, she will storm out the room and throw things with behaviors lasting about 30-60 minutes in duration. Intervention: QMHP assisted client in monitoring her own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. QMHP modeled proper positive behavioral expectations for the client. QMHP introduced concept of nonverbal communication cues in efforts to review the expected behaviors during pandemic and the appropriate way to talk about her mental illness with her family. Intervention: QMHP encouraged the client to role play appropriate and inappropriate conflict resolution skills in efforts to highlight and demonstrate several appropriate options the client may utilize when she feels challenged or the need for impulsive disruptive actions. QMHP provided strategies such as positive reframing. QMHP assisted client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations that usually trigger the client's negative behaviors. Response: Client was displaying respectful, non-aggressive behaviors in the home. Client expressed positive thoughts about herself. Client stated she didn't need help with anything at the moment but would tell an adult if she needs help. Client communicated by nodding and exhibiting good eye contact. Client verbalized positive feelings about goals and objectives. Client understands that her treatment plan is a behavior contract that will reflect her improvements and setbacks in the home and community settings. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Response: Client was acceptive of support and encouragement. Client exhibited good social behaviors. Client was able to reevaluate herself and display a more positive attitude toward peers and authority. Client has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Client was able to communicate with the QMHP as information was shared. Client communicated that she felt safe and comfortable talking with the QMHP. Client shared that she enjoyed the session she had with counselor.
